If you are uncertain as to which of our studies would be the appropriate diagnostic tool for your patient, please go
to our online imaging request and choose the procedure for consultations with radiologist for appropriate study. Fill
out the request and submit it. A radiologist will contact you to discuss your case and the options available for the
best diagnostic study.
If you elect not to submit your request online, please choose and download one of the printable imaging request procedures
(pdf forms) below, print it and fill out the required answers. Then fax it to Veterinary Medical Center Advanced Veterinary
Imaging Direct at (612) 624-8779.
Please Note:
As this procedure is being performed on an outpatient basis, you will remain the primary attending veterinarian,
unless you or your client specifically request consultation with one of our specialty services. As the attending
veterinarian, the program therefore requires that you be available for consultation regarding medical and diagnostic
decisions throughout the time the patient is at the Veterinary Medical Center.
